Some meals have different effect instead of giving 1 Blue Heart: Ritualistic Fight Pit lets you pin two Followers against each other in a fight to the death. Once a day, eat a meal to receive 1 Blue Heart. Alternatively, walking down to the ritual circle will skip the option and just result in a death as normal.If the Follower is a spouse, you will receive additional Blue Hearts equal to the number of Red Hearts, allowing for a maximum of 4 Red Hearts and 4 Blue Hearts to be received. If the Follower is an elder, you will receive half a Red Heart. If the Follower is sick or a dissenter, the Red Hearts are halved (capped at 2 Hearts). You will be resurrected with the amount of Red Hearts equals to the Follower's level (capped at 4 Hearts). You can choose 1 Follower to be sacrificed. When killed the first time on a crusade run, sacrifice a Follower to be resurrected. Use 1 Heart of a Heretic to unlock new abilities. Use a Commandment Stone to declare a new Doctrines, which unlock Rituals or Traits for your cult.
